Community Health Representative
Position: Community Health Representative (CHR)
Department: Community Health Representative Program

Job Purpose
The Community Health Representative (CHR) provides community-based health services focused on improving the overall health status of tribal members through outreach, education, prevention, and basic health services. This position serves as a liaison between the community and healthcare providers, delivering culturally appropriate care and support to individuals and families, including elders and children.

Essential Duties and Responsibilities
• Conducts home visits to assess client health needs, living conditions, and overall well-being.
• Assists with basic health screenings, including vital signs and general health assessments.
• Provides health education related to disease prevention, chronic disease management, nutrition, and healthy lifestyles.
• Assist clients with activities of daily living (ADLs) when appropriate.
• Supports clients with chronic conditions, including diabetes care (e.g., foot checks, blood glucose monitoring assistance).
• Provides guidance on the use of medical equipment such as walkers, wheelchairs, and canes.
• Serves as a liaison between patients, families, and healthcare providers to improve access to services.
• Assist clients by scheduling appointments and completing required forms for services.
• Transports clients to non-emergency medical appointments as needed.
• Advocates for clients by explaining available health programs, services, and policies.
• Refers clients to appropriate healthcare providers and community resources.
• Participates in community outreach, health fairs, and prevention programs.
• Assists with health clinics, immunization events, and community education initiatives.
• Accurately documents services and enters patient data into RPMS or other required systems.
• Maintains patient records in accordance with program policies and confidentiality standards.
• Attending courses require training, meetings, and conferences to maintain CHR certification.
• Promotes cultural awareness and incorporates Native American traditions and language when appropriate.
• Performs other duties as assigned.

• Ability to communicate effectively with individuals from diverse backgrounds.

• Ability to recognize health concerns and report them to appropriate medical personnel.
• Strong organizational skills and attention to detail in documentation and reporting.
• Ability to work independently and manage time effectively.
• Ability to maintain confidentiality of sensitive patient information.
• Cultural competency and respect for Native American customs and traditions.
• Ability to handle difficult situations with professionalism and tact.
• Basic computer skills, including data entry and recordkeeping.

Minimum Qualifications
• High School Diploma or GED required.
• Community Health Representative (CHR) certification preferred or must be obtained within a specified timeframe (e.g., 6–12 months).
• CPR and First Aid certification required (or must be obtained within 60 days of hire).
• Minimum of six (6) months of experience in healthcare, social services, or a related field preferred.

Licensing Requirements
• Valid driver’s license and must be insurable under Seminole Nation policy.
• Must successfully pass a background check in accordance with Tribal policies.

Work Environment
• Work is performed in a combination of office, community, and home settings.
• Frequent travel within the service area is required.
• Exposure to environmental conditions, contagious illnesses, and moderate noise levels.

Physical Demands
• Ability to stand, walk, sit, bend, and move for extended periods.
• Ability to lift and carry light to moderate weight (up to 25 pounds).
• Ability to navigate uneven terrain and perform duties in various environments.
